Community Health Needs Assessment Survey
We are asking for community input as we are currently conducting the Mahoning and Trumbull County Community Health Assessment. Mercy Health and public health agencies in Mahoning and Trumbull Counties, with support from Moxley Public Health, are conducting the survey to learn more about the health and topics of concern among its residents.
We want to hear from all people living in Mahoning and Trumbull Counties in this survey.
Your voice and the information you provide is valuable to the work we do, and we value your input! We will use the results of this survey to help create action plans to address major community health issues in Mahoning and Trumbull County. We truly appreciate and NEED your help on this! We want to be able to prioritize the health issues that are most important to community members, and we need this survey to do that.
