Community Health Needs Assessment Survey
It is again time to complete our large-scale Community Health Needs Assessment survey with Mahoning and Trumbull County. This is a process health department must complete every three years to gather information for our Community Health Improvement Plan (CHIP). Last cycle we had incredibly low response rates for Youngstown City residents. Only 275 residents took the previous survey, which is less than 0.01% of our population. We would like our residents' voices to be heard so we can further set goals to improve the health and wellbeing of all.
Attached is this cycle Needs Assessment Survey in both English and Spanish. There are also feedback QR codes to help us improvement this process for future surveys. We would like this survey and QR code to be at ALL Health Department Events moving forward. We would also like all of the CHWs to have copies of these with them at their offices and when they are out doing outreach events. There are some printed copies on the table outside of Faith's office.
