Affidavit of Correction of Birth Record forms are prepared at no charge in accordance with the Ohio Department of Health Office of Vital Statistics.

An Affidavit of Correction is used to correct minor spelling and data entry errors on a birth certificate (e.g. misspelling of child’s name, mother’s name, father’s name, date of birth, etc). They cannot be used to change major items, such as race, gender, or year of birth. The Vital Statistics Division submits your completed and notarized affidavit to the State Office of Vital Statistics for review and determination. If approved, the corrected birth certificate will be available for purchase on the same day.

For any major changes or corrections you will need to go through your local probate court .

If you notice an error on a death certificate please contact the funeral home to have them file an affidavit to correct the certificate.

Efiling

Scan the completed death certificate, as well as the Request for Approval Form (see below), and email to efile@youngstownohio.gov. Please include the name of the decedent in the email subject line. Currently, only death certificates can be filed; affidavits and physicians supplements must still be brought in or mailed to the Vital Statistics Division office (see location below).

In Person

Bring the certificate for review and registration to the Vital Statistics Division (see location below). Please allow ample time for the review and registration of each death certificate.

By Mail

Send the completed certificate to the Vital Statistics Division (see address below), with a business check and your application. Do not fold the certificate as we are unable to accept folded death certificates, affidavits, or physician’s supplements.

Do not use this portal to submit death certificates; it is only to be used for purchases. Vital Statistics Division Assistance

Should you have a limited number of records and can supply enough information to locate the record, we will be happy to check for availability of the record for viewing or purchase. Non-certified copies of vital records are no longer available. The following information is needed in order to conduct a search:

• Name on the certificate
• Date of the event (year, if the exact date is not known)
• City or township of the event (especially needed for older records)
• Parents' names, including mother's maiden name

Statewide Index Search

Come into the Vital Statistics Division office to search the Ohio Department of Health Statewide Index for basic birth and death information. Birth information is from 1908 to the present and death information is available from 1964 to the present. No appointment is necessary.

An Acknowledgement of Paternity Affidavit allows unmarried parents to name the natural father and establish paternity for their child. This voluntary form must be signed by both parents. Acknowledgement of Paternity Affidavit forms are prepared at no charge.

• If the paternity of the child is unknown, an Acknowledgement of Paternity Affidavit should not be completed. Rather, contact the Mahoning County Child Support Enforcement Agency at (330) 740-2073 for more information about its Paternity services. Genetic testing is available.
• If the natural father was unavailable to sign the Acknowledgement of Paternity Affidavit at the hospital of birth, please contact the Vital Statistics Division for assistance.
