Survey - Community Health Needs

The City of Youngstown participated in the Mahoning and Trumbull Community Health Assessment in 2022. Unfortunately, only 275 responses were submitted by City residents, representing less than 1% of a population of 60,000, making it impossible to truly address the health needs of City residents.

The Youngstown City Health District, in conjunction with the Ohio Department of Health, has set up Health Improvement Zones (North Side, South Side, East Side, West Side) dedicated to our City residents. The first step in the process to bring change is to conduct a Needs Assessment, specific to the City of Youngstown. This survey will provide the Health Department with Youngstown-specific data on what residents truly need to improve their health and well-being. This data will then be used to create interventions specific to the priorities identified in each zone.

We are asking all residents to take a moment to take our Needs Assessment Survey (see links below). We want to hear what residents need in their neighborhood. This 5-minute survey is the voice for all residents for future plans to improve their health and well-being.
